Registration Form (Employer and Kasambahay)

Reminder: Fields marked with ( * ) are required. Please complete all mandatory fields before saving the employer record.

1. Fill Out Employer Details

Enter the employer's First Name, Last Name, Middle Name (if applicable), and Suffix (Jr., Sr., III, etc.).

2. Enter Address & Contact Information

Select the Province, Municipality/City, and Barangay. Enter the complete address (House No., Street, Purok/Sitio) and provide an active telephone or mobile number.

3. Upload a Valid ID

Upload a clear image of the employer's valid government-issued ID by clicking the Browse button.

4. Add Kasambahay Information

Click + Add Kasambahay, then enter the domestic worker's Full Name, Telephone Number, Type of Work, and Monthly Salary. Save the information to add it to the list.

5. Review & Save

Review all information before submitting. Ensure all Kasambahay records have been added correctly, then click Save Employer. Use Clear to reset the form if necessary.
